CONSENT WAIVER AGREEMENT FOR SERVICES

This Consent Waiver Agreement (the “Agreement”) is made and entered into as of today, by and between David Lee Williamson (“Provider”), and the undersigned client ("Client").

WHEREAS, Provider offers life coaching and energy healing services; and

WHEREAS, Client desires to receive such services from Provider; and

WHEREAS, Provider is willing to provide such services only upon Client's understanding and acceptance of the terms and conditions set forth in this Agreement; and

WHEREAS, in the event that Client is a minor (under the age of 18), a parent or legal guardian ("Guardian") must execute this Agreement on Client's behalf and accepts all terms herein on behalf of themselves and the minor Client.

NOW, THEREFORE, in consideration of the mutual covenants contained herein, and for other good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, the parties agree as follows:

1. Services. Provider agrees to provide life coaching and/or energy healing services to Client, as agreed upon between the parties. Client understands that these services are for the purpose of holistic wellness and are not intended as a substitute for medical or psychological care.
2. Nature of Services. Client understands that the energy healing session involves a method of energy balancing. Client understands that Provider may or may not apply light touch if in- person work is provided.
3. No Medical Advice. Client acknowledges that Provider does not diagnose conditions, prescribe medicines, or interfere with the treatment of a licensed medical professional. Client is advised to seek a licensed health care professional for any physical or psychological ailment.
4. Confidentiality. All sessions provided by David Lee Williamson will be recorded as a Zoom meeting or any other recording format for the protection of both parties. As a client, Client understands and agrees that Zoom meeting recordings will be made available to the public as teaching and or healing tools for others to access. Client understands that they may choose to use an alias and keep their camera off during the session if they wish anonymity.
5. Client Responsibility. As a client, Client understands and agrees that Client is fully responsible for Client's wellbeing during coaching sessions, including Client's decisions and choices. Client may discontinue coaching sessions with Provider at any time. Client understands that coaching is not psychotherapy, counseling, or any medical alternative and will not use it in place of any form of therapy or medical advice. Client agrees that Client is well-adjusted, mentally healthy, and ready for coaching. It is Client's responsibility to consult Client's health care provider prior to participating in coaching with Provider. Should Client choose not to consult Client's primary health care provider, Client accepts full responsibility and waives all rights to liability or any claims against Provider or any affiliated administrators, or employees.
6. Release and Indemnification. Client agrees to release, indemnify, defend, and hold harmless Provider, its owners, employees, agents, and contractors from and against any and all claims, damages, losses, liabilities, costs, and expenses (including reasonable attorneys' fees) arising out of or relating to the service(s) provided by Provider; Client's own acts or omissions; Provider's employees, agents, or contractors; and any third-party claims arising from or in connection with the service(s). This obligation to indemnify applies regardless of whether such claims are caused in part by the negligence of Provider, except where prohibited by law.
7. Perpetual Agreement. This agreement is intended to be perpetual and will remain in effect for all future services provided by Provider to Client unless otherwise terminated in writing by either party.
8. Governing Law and Venue. This Agreement shall be governed by and construed in accordance with the laws of the State of Texas. The parties agree that any disputes under this Agreement shall be resolved in the courts located within Texas.
9. Entire Agreement. This Agreement constitutes the entire understanding between the parties regarding the subject matter hereof and supersedes all prior oral or written agreements.
10. Cancellation Policy. Client understands that Client is responsible for the fees if Client cancels less than 24 hours before a scheduled session. Missed sessions will not be made up.
11. Voluntary Participation. Client is choosing to participate in coaching services with Provider. Any suggested Yoga Poses, Meditation, or Reading Material is just a suggestion. It is Client's choice to perform or read any suggested material, and Client does them at Client's own risk.
12. Attorney Fees. Client agrees that Client would be responsible for attorney fees for all parties if a lawsuit is filed.
