EU Adults · 18 & Over
You are an EU resident scheduling a session for yourself as an adult client. This form collects your health background and session focus, with full GDPR-compliant data processing disclosures and your explicit consent.
EU Residents · Minor Under 18
You are an EU-resident parent or legal guardian scheduling a session for someone under 18 years of age. This form includes guardian authorisation, parental consent, and GDPR-compliant data disclosures specific to minors.

Right of Access
You can request a copy of all personal data David holds about you, at any time.
Right to Rectification
If any information held is inaccurate or incomplete, you have the right to have it corrected.
Right to Erasure
You can request that your personal data be permanently deleted ("right to be forgotten").
Right to Restrict
You can ask that processing of your data be paused while any concern is resolved.
Right to Portability
You can request your data in a machine-readable format to transfer to another service.
Right to Object
You can withdraw consent and object to processing of your data at any time.
